Our staff will provide you with forms that are ready
to send by mail to your insurance company. These forms will show the charges for the services performed and
the diagnosis treated. We will explain everything that you need to know to easily submit your claims. We welcome patients who do not have insurance and believe that you’ll find our fees to
be reasonable. We do not want cost to limit the care you need to receive. Please contact us with any questions you might have
